Consent and Signatures

The terms and conditions of the Western Conference of Teamsters Legal Services Trust Plan ("Trust"), Summary Plan Description, and Panel Attorney Agreement are incorporated by reference and made a part of this document.

Client has the right to terminate the service of Panel Attorney at the any time upon written notice. If Panel Attorney is attorney of record in any proceeding. Client agrees to promptly execute and return a substitution of attorney form. Panel Attorney has the right to withdraw for any reason for which withdrawal is authorized or required by the applicable Rules of Professional Conduct.

Services are provided by Panel Attorneys pursuant to the Trust, Summary Plan Description, and Panel Attorney Agreement. Panel Attorney's legal fees will be paid only by the Trust, and in no event shall Participant or Client be asked to pay Panel Attorney's legal fees or any portion thereof. Because certain costs, including the cost of title searches, deed mortgage recording, private investigations, and medical tests among others, are not covered by the Trust, they will be charged to the Participant.

THE CLIENT CONSENTS TO THE RELEASE BY THE PANEL ATTORNEY OF ANY INFORMATION THAT MAY BE REQUESTED BY THE WESTERN CONFERENCE OF TEAMSTERS LEGAL SERVICES TRUST FUND'S MANAGER, INDEPENDENT AUDITOR, OR COUNSEL. THIS INFORMATION INCLUDES ALL DOCUMENTS IN THE PANEL ATTORNEY'S FILES PERTAINING TO THE MATTER DESCRIBED IN SECTION 4.

NOTICE OF RIGHTS AND RESPONSIBILITIES

Member Must Read & Sign The Trustees of the Western Conference of Teamsters Legal Service Plan are pleased to provide a program of pre-paid legal services. This outlines some of your rights and responsibilities under the Plan.

Rights:

1. To pay no fees in connection with your representation. The Plan pays all your attorney's fees; you may not be required to pay additional fees.

2. To pay only for certain costs not paid by the Plan. The plan pays up to $600 per year towrd filing costs for service subpoenas. You may be required to pay for the filing and service costs that exceed that amount, and also costs for investigators, expert witnesses, and non-lawyer consultants. You may or may not be required to pay for postage, copies or fax charges. You may or may not be required to pay delivery costs if a special request by you requires express delivery.

3. To have the attorney complete work on your case, even if the maximum benefit has already been paid by the Plan.

Responsibilities:

Efficiency is the key to an effective legal services plan. We urge Panel Attorneys to provide their services as efficiently as possible. As the client, you also have a role in helping assure that services are provided efficiently. Your attorney should keep you up to date regarding developments in your case. Because Panel Attorneys have numerous clients, it is often more efficient to update you in writing, or by sending you copies of the pleadings and letters, than to discuss events with you on the telephone. If that is your attorney's practice, we urge you to cooperate in the arrangement.

Of course, if you feel you are not receiving adequate service, you should ask your Panel Attorney to address your concerns. The trust cannot guarantee the quality of representation you receive from the Panel Attorney. But if your attorney fails to address your concerns, you may notify the Trust Office at the address : PO Box 2340, Stockton, CA 95201

The Trustees sincerely hope that you receive excellent legal service through the Plan. To assure that Panel Attorneys are providing quality services in an efficient manner, The Trustees routinely send questionnaires to participants. If you receive such a questionnaire. please complete and return it, to give the Trustees an accurate sense of the quality of service your Panel Attorney provided.
